You will find that the call centre officer will ask few questions to verify your details, it would be convenient for you if you have these details at hand before you contact the call centre:

  • Your policy information; vehicle no/policy no, name of the insured.
  • Information about the incident, such as the time, where the damage occurred, etc.
  • Details about the loss or damage you are claiming for
  • Information about any other people, vehicles or property involved

Life Insurance

Following documents are required for settlement of a Life Claim

Death Claim

  • Completed claims form signed by the nominee/legal heirs
  • Original Death certificate
  • Last medical attendance certificate & hospital treatment certificate
  • Inquest proceedings
  • Postmortem reports
  • Past medical history reports
  • Policy document & 1st schedule
  • Birth certificate of the deceased
  • Birth/ marriage certificate of the nominee/legal heirs

Personal Accident Claims

  • Claimant's statement
  • Medical attendance report to be completed by the consultant/surgeon who treated the patient
  • Diagnosis tickets, X-ray & medical investigation reports etc. pertaining to the accident
  • Leave certificate by the employer in case of weekly benefit claims

Critical Illness Claims

  • Claim Form
  • Private medical attendance report
  • All diagnosis tickets & medical reports pertaining to the illness
  • Policy document & first schedule
  • Other reports relevant to particular illness (will depend on the illness)

Hospitalization Claims

  • Claim Form
  • Original diagnosis ticket & all medical records
  • Original bill & prescription if any
  • Reports of the medical history
